This form is used to request a new district-issued mobile phone, report a lost, stolen, or broken device, or request an upgrade or reassignment. It must be completed with all required user, department, and authorization details, then submitted to Technology Services via email. Phones are district property and must be returned when no longer in use. Director or CBBO approval is required for certain titles.
